As a participant in this project, I have read the letter of information, had my questions answered to my satisfaction, and understand the general nature, purpose and procedures of the project as explained to me by the researcher.

I understand that data resulting from my participation in this study will not identify me in any way. I understand that I will be assigned a pseudonym when the interview is transcribed onto paper and only the name of the reservation I live on will be attached to my name. I also understand that I may withdraw from this study at any time during the interview, for any reason, and request that my interview not be included in the research. Once I leave the interview room, I understand that I may no longer withdraw from the study.

I hereby give my permission to be interviewed. I understand that the interview will be audiotaped, and that the tape recordings, interview transcripts, and field notes may be used for the purposes of graduate papers at the University of Alberta, journal articles, and/or conference presentations/posters.

The plan for this study has been reviewed for its adherence to ethical guidelines and approved by the Faculties of Education, Extension, Augustana and Campus Saint Jean Research Ethics Board (EEASJ REB) at the University of Alberta.
